Notification

If you would like to lodge a travel claim you are kindly requested to notify us immediately of your loss/damage by completing and returning a claim form to travelclaims@middlesea.com or by visiting our offices.
You can also log into MyInsurance and register your claim by inserting your policy number.

Standard Documents Applicable for All Sections

To submit any travel claim please present the following:

  • Flight / E -tickets / Boarding Pass
  • Policy Schedule
  • Copy of passport / ID card
Cancellation or Abandonment

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Hotel Accommodation vouchers/receipts
  • Flights invoices and receipts
  • Tour/holiday programme
  • Documentation to support reason/s for cancellation or curtailment: Medical report/s, Death certificates and/or other supporting documentation
  • Refund confirmation for unused flights etc
  • We may request insured to call at our medical consultant for an examination or to provide further information

In the event of cancellation you are kindly asked to notify the travel agent/ticket issuing office immediately and apply for the refund of taxes and other dues.

Trip Cancellation due to extreme weather conditions & natural catasrophes

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Refund confirmations
  • Details of pre-paid expenses for travel and accommodation which you cannot get back
  • Supporting evidence
Emergency Medical Expenses & Treatment

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Medical Report/s and medical invoices and/or receipts
  • Hospital reports and hospital invoices and/or receipts (In-patients hospital treatment or surgery is to be notified and authorized by MAPFRE Middlesea plc)
  • Refund Confirmation for unused flights etc
  • Receipts for additional accommodation and traveling
  • We may request insured to call at our medical consultant for an examination or to provide further information

Delayed Departure

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Boarding Pass for Actual Departure
  • Reason for delay sustained duly supported by documentation (Airline declaration/Newspaper cutting etc)
Missed Departure

To submit a claim please present the following:

  • Travel claim form completed in full indicating clearly amounts claimed and form duly signed
  • Airline’s declaration to confirm refunds for the unused flights
  • Receipts for extra charges incurred
  • New flight ticket
  • Supporting evidence & prove why departure was missed
Personal Money

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Police Report filed within 24hrs of loss
  • Bank Statement/s confirming amount of local cash withdrawn and foreign currency exchanged
  • Claimants may be requested to make a sworn statement (affidavit)
Baggage (Personal Belongings)

To submit a claim present:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed.

Damaged Suitcase

  • Original Damage Report by Airline, Original Baggage Tags/Original Baggage Check Receipts
  • If a report is not filed at the airport, insured is to lodge a report in writing with the Airline within seven days from date of flight

Non-Delivery of Suitcase

  • Original Property Irregular Report
  • Airline Baggage Check Receipt

 List of Missing items 

  • Receipts/Visa Statements/Valuations/Manuals/Accessories of missing items(if available)
  • Claimants may be requested to make a sworn statement (affidavit)

 Stolen Items:

  • Police report or Airline report or Hotel report within 24hrs of loss
  • Receipts/Valuations/manuals/accessories of stolen items (if available)
  • Quote/s replacement of same or similar item/s
  • Claimants may be requested to make a sworn statement (affidavit)

 Delayed Luggage 

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Original Property Irregularity Report
  • Receipts of suitcase when collected/returned or similar documentation
  • Original baggage – check Receipt
  • Original receipts for emergency expenses incurred

Travel Documents including Loss of Passport

  • Travel Claim Form completed in full indicating clearly amounts and form duly signed
  • Police Report made within 24 hours of your loss
  • Receipts and documentation to support extra expenses

Accidental Damage to Personal Belongings

  • Technician’s Report
  • Receipt for Repair Cost

In case item cannot be repaired:

  • Damage item and accessories
Personal Accident

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed Medical Report/s
  • Insured may be examined by a medical consultant appointed by MAPFRE Middlesea plc
Your Liabilities to Others

To submit a claim please present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Medical Report/s and medical invoices and/or receipts
  • Other documentation to sustain insured claim depending of type of claim
  • Photos of damages
  • These claims are to be handled directly by MAPFRE Middlesea plc
Rental Vehicle Policy Excess

To submit a claim present the following:

  • Travel Claim Form completed in full indicating clearly amounts claimed and form duly signed
  • Rental Agreement
  • Photos of damages
  • Original Receipts
